Under certain circumstances, a man can develop fibro-fatty breast tissue that resembles a woman’s breast. This condition is referred to as gynecomastia. It is typically symmetrical enlargement of the breasts. The etiology of this process is not widely understood; however, it is known that certain medical conditions, medications, hormones, drugs, and other factors may play a role in its development. In the majority of cases, no etiology can be determined. Depending on the extent of the problem, gynecomastia may be treated with excision of the breast bud, liposuction of the fatty tissue, or a combination of both procedures. Occasionally, a secondary procedure to reduce excess skin or to return the nipple to a more normal position may be needed. Results can vary, but generally patients have a chest that is flatter, firmer, and better contoured, and they often feel more comfortable in form fitting clothing or going without a shirt.

If you are a male with persistent enlargement of your breasts that cause you to be self-conscious, you are a good candidate for male breast reduction. Using a combination of techniques including liposuction and direct excision, Dr. Greenwald is able to obtain natural looking results with minimal down time. During your initial gynecomastia (male breast reduction) consultation, Dr. Greenwald will inquire about your medical and surgical history including current medications and allergies. It is important to be thorough when discussing your medical history as safety is Dr. Greenwald’s top priority. Your full disclosure of all pertinent medical information prior to surgery will reduce the incidence of unwanted complications after surgery. You will discuss your concerns and motivations for seeking breast augmentation. Details of gynecomastia will be discussed including incision placement as well what is expected of you preoperatively and postoperatively. Depending on the characteristics of the tissues, gynecomastia surgery may entail a liposuction only technique. Often times, excision of the dense breast tissue under the areola is needed through an incision at the junction of the areola and normal skin (periareolar incision). It is very uncommon to have to make more than the periareolar incision at the first operation. If a patient has significant skin excess, it is possible that additional skin ma need to be removed at a second operation if there is not enough skin contraction after the breast liposuction. The risks of surgery will also be discussed in detail. Numerous before and after photographs of actual patients will be reviewed during your consultation.

You will then be asked to put a gown on. Dr. Greenwald will then make several important measurements. Photographs will be taken. Greenwald will then review your photographs with you. Differences between the 2 breasts (i.e. asymmetry) will be noted and proposed incisions demonstrated. Computer imaging is an invaluable part of the consultation. Dr. Greenwald encourages you to ask any questions during you consultation. Following your consultation, you will meet with Missy, Dr. Greenwald’s patient coordinator, to discuss scheduling your surgery and answer any questions you may have forgotten to ask Dr. Greenwald. If indicated (firm gynecomastia, unilateral gynecomastia), preoperative imaging including a mammogram and an endocrine workup will be performed.

Gynecomastia surgery is not an exact science. Although the majority of men experience no complications, Dr. Greenwald will discuss potential risks and complications of male breast reduction surgery at the time of your consultation.   • What results can I expect after my gynecomastia surgery?

Provided you are a good candidate for male breast reduction surgery, the contour of your chest will be significantly improved. The results will be long lasting. Scars will fade over time and Dr. Greenwald will tell you how to care for you incisions. If liposuction was a significant part of your male breast reduction surgery, swelling in your chest will resolve over three to six months.
